Who is Eligible?

A child with a critical illness who has reached the age of 2½ and is younger than 18 at the time of referral is potentially eligible for a wish.

After a child is referred, Make-A-Wish® will work with the treating physician to determine the child's eligibility for a wish, i.e. suffering from a progressive, degenerative or malignant condition currently placing the child's life in jeopardy. 

Our vision is to grant one wish to every eligible child. While more than 15,300 wishes were granted across the United States last year alone, this reached only half of the potentially eligible children diagnosed with a critical illness.

How can I refer a child?

To refer a child in the state of Arizona, please contact Jennifer Fleming at 602.343.9446 or by email at wsyrzvat@nevmban.jvfu.bet.  To learn more about referring a child who lives outside of Arizona in the United States or one of its territories, you may use our online inquiry form. The inquiry form is not an official application, but a request for more information.

If you would like to learn more about referring a child residing outside the United States, please contact Make-A-Wish Foundation International. If you are not eligible to refer a child, please ask the child’s family to visit our website or contact their local chapter or international affiliate.
